What you should bring
- Your insurance card along with another form of I.D. (i.e. – Driver’s License, State ID card, Military ID)
- X-Rays, MRIs or other pertinent medical records
- Information about prescription medication which you may be taking
- Cash, Checks, Credit Cards (Visa, MasterCard, American Express)
New Patient Forms
We realize filling out paperwork for healthcare in today’s world is no picnic. We have made every effort to minimize the paperwork required for our patients, while also getting the information we need to provide you with the highest quality service possible. In order to make better use of your time at our locations, we have provided you with registration forms to be printed and completed prior to arriving at your first appointment. The forms included in this registration packet enable us to learn about your condition and how it affects your daily life, understand your overall health, track the effectiveness of your rehabilitation, and accurately bill your insurance.
